Summer Staff Applications
Select the desired application form from the list below and print it to your copier. Applications are also available by either calling the Camp Lebanon office or e-mailing Linda Day, Program Director, at LDay@CampLebanon.org.
* All applications must be accompanied by references.

In addition to your application please submit one completed Staff Reference Form from a church worker who knows you well (e.g. youth pastor, Sunday school teacher, youth group leader) who is not related to you.

Applicants must have three written reference forms from:
- Spiritual Leader (pastoral reference).
- Employer.
- Professor or adult friend who knows you well.
