Registering for a Camp Lebanon event is as easy as one, two, three!

Click on the desired event from the list below.
Print as many copies as you need. (Note: One person and one event per form for youth and adult events.)
Mail the completed form with payment to the address indicated on the form. The registration forms are in Adobe Acrobat PDF format and require Adobe Acrobat Reader to view or print.
